Lately, I've been waking up in the morning drenched with sweat. I'm not sleeping in wool or flannel, and I don't think I have a fever. What could be causing this?

Nov. 13, 2000 -- Your night sweats could be caused by a number of factors. First, your choice of sleeping attire, even if it's not wool or flannel, could be causing you to sweat. Synthetic fabrics -- for instance, polyester or nylon -- can trap heat and keep your skin from cooling. Your bed sheets, if made of these fibers, also could be the source. Go with 100% cotton on all sleepwear and sheets.

Another reason for your night sweats may be -- you might have guessed -- hormones, which can cause sweats as a result of marked changes in progesterone. Before menstruation and during ovulation the level of progesterone rises, making your basal temperature go up, causing sweats in some women. Similar symptoms also can happen before and during menopause.

Hormone-related sweats in nonmenopausal women are fairly easy to identify, however, because they are more like hot flashes -- that is, they tend to start in the face with a burning feeling and then sweep down the body. If you're concerned that your sweats may be due to hormones, discuss the possibility with your doctor of doing blood tests on the second or third day of your menstrual cycle to check your hormone levels.

Certain drugs also can cause night sweats. Common culprits include selective serotonin reuptake inhibitors (such as Prozac, Zoloft, and Paxil) and other drugs for the treatment of depression. Talk to your doctor about taking a break from the drug(s) to see if the problem goes away. Alcohol, if consumed within an hour of going to bed, also can cause you to dampen the sheets while you sleep.

On the rarer, more serious end of the spectrum is tuberculosis (TB), an atypical bacterial infection that usually starts in the lungs. Common symptoms are coughs and night sweats. TB typically is spread through populations living in crowded and unsanitary conditions, such as mental institutions, military camps, and dorms. You can find out if you have TB by going to your doctor and getting a skin test and possibly a chest X-ray.

Sweating caused by other diseases, such as Hodgkin's disease (a lymphoma), tend not to be confined to the evening. In fact, most other diseases that can cause sweats usually are accompanied by other serious symptoms, such as weight loss, so if you're otherwise feeling fine, you're probably in the clear.

Finally, even though you've said that you do not think you have a fever, it might be a good idea to take your temperature anyway so you can provide this information to the doctor and help rule out the more serious possibilities. Try keeping a thermometer by the bed, so that if you wake up drenched, you can pop it in without having to stumble off to the bathroom.
