For about a week before my period, my breasts get swollen and sore. What can I do to stop this?

Sept. 4, 2000 -- You don't have to suffer the swelling every month. In fact, there are a number of ways to treat it. Breast tenderness is a common symptom of a condition most women know and love: PMS. Premenstrual syndrome occurs during the last two weeks of the menstrual cycle, right before the period begins. During this time, a woman's levels of the hormones estrogen and progesterone rise. This causes increased blood flow to the breasts, water retention, and inflammation and swelling of breast tissue ducts and glands -- all of which can leave you feeling sore in the breast area.

Luckily, there are many ways to deal with the problem, depending on how severe your symptoms are. For most women, breast tenderness due to PMS comes and goes in cycles -- some months tend to be worse than others. By experimenting with a variety of therapies -- including the five simple ones listed below -- most women can get the soreness under control.

1. Say goodbye to coffee and other caffeinated beverages after you ovulate at mid-cycle (for most women, this is around the 14th day of their cycle). Caffeine tends to increase both swelling of the breasts and water retention.

2. Consider taking a multivitamin and calcium daily. The jury's still out on whether certain vitamins and minerals can really help with PMS symptoms -- a study published in the February 2000 Journal of the American College of Nutrition was lukewarm on the true effectiveness of vitamin E, vitamin B-6, and calcium. But some intriguing research suggests that these nutrients may help remove water from the body and alleviate swelling in the breasts.

3. Consider evening primrose oil supplements. Again, there are no well-controlled studies to confirm that this supplement works, but some women seem to find relief with primrose oil's anti-inflammatory properties.

4. Try an over-the-counter medication, such as Pamprin or Midol, that treats the symptoms of PMS. These medicines contain pain relievers and natural diuretics, which can alleviate discomfort.

5. If nighttime is especially uncomfortable, try sleeping on your back to protect your breasts from the painful compression you might feel when sleeping on your side or front.

If, even after following these steps, you continue to suffer from a severe case of breast soreness and PMS, visit your gynecologist, who may be able to reduce your breast tenderness through hormone manipulation. Progestin-based birth control pills are a popular choice. Called "minipills," they do not contain estrogen, which many experts believe is the main culprit behind PMS symptoms.

Two other prescription drugs, Danazol and Spironolactone, may also help. Both have estrogen-blocking properties, and Spironolactone, a diuretic, helps remove excess water from breast tissue, which can alleviate some of the PMS-related soreness.
